Linking an Individual Account to a Healthcare Provider

Please read this consent carefully. This Consent for Linked Accounts (this “Consent”) explains what happens when you choose to link your individual SkinSAFE account to the account of your healthcare provider on the SkinSAFE Provider Portal, and asks for your permission to share certain information with your provider through the SkinSAFE services. This Consent is part of, and should be read together with, the SkinSAFE Consumer Terms of Service and the SkinSAFE Privacy Policy. Your provider’s use of your information through the Provider Portal is also governed by the SkinSAFE Provider Portal Terms of Service and the SkinSAFE Business Associate Agreement (the “BAA”) between SkinSAFE and your provider.
If your healthcare provider uses the SkinSAFE Provider Portal, SkinSAFE can work with your provider to help identify dermatological products and therapies that may be appropriate for your individual condition and characteristics. To do this, SkinSAFE uses the health information you enter into the SkinSAFE services together with information SkinSAFE receives from your provider (such as your patch test results and allergen sensitivities) to generate a list of products and therapies your provider may consider as part of your care.
When you link your account to your provider, the following may be shared between you, your provider, and SkinSAFE to support your care:
The information your provider inputs and manages about you through the Provider Portal is “protected health information” and is handled in accordance with the BAA and applicable law, including HIPAA. SkinSAFE’s handling of the information you provide directly is described in the Privacy Policy.
By clicking “I agree” (or a similar control) and linking your account to your provider, you consent to the collection, use, and sharing of your health information — including allergen and patch test information — between you, your healthcare provider, and SkinSAFE through the SkinSAFE services for the purpose of supporting your care, and you acknowledge and agree to the terms of this Consent. The information shared in this way will be used to generate a list of products and therapies your provider can use as part of treating your dermatological condition(s).
You understand that, in some cases, SkinSAFE may receive payment from the makers of products or therapies that appear on the list generated for your provider. This does not change your provider’s independent clinical judgment, and the final decisions about your care are made by you and your provider.
SkinSAFE may also use information you or your provider share to operate and improve its services and to help develop new products and therapies. Any information used or disclosed for these purposes will be de-identified so that it does not identify you and cannot reasonably be traced back to you, consistent with the BAA and applicable law.
Linking your account and providing this Consent is voluntary, and you may decline. You may withdraw this Consent at any time by unlinking your account in your settings or by contacting us at privacy@skinsafeproducts.com or contacting your provider. Withdrawing your Consent will stop future sharing of your information through the linked-account feature, but it will not affect (i) actions already taken in reliance on your Consent, (ii) information already shared with or held by your provider, which remains subject to your provider’s obligations and the BAA, or (iii) de-identified information. Withdrawing this Consent may limit your ability to use the linked-account feature, but it does not by itself terminate your individual SkinSAFE account.
This Consent supplements, and does not replace, the Consumer Terms of Service, the Privacy Policy, and (as between SkinSAFE and your provider) the Provider Portal Terms of Service and the BAA. To the extent this Consent conflicts with the BAA regarding protected health information, the BAA controls.
